Please fill in the fields of this form to register for sacrament preparation at Holy Name Parish.

If your child was baptized at Holy Name Parish, please complete the Baptism information section – but we will not require a copy of the Baptism certificate. If he or she was baptized elsewhere, we will need a copy of the Baptism Certificate. The copy can be faxed to us at (403) 249-9952, emailed to sacraments.holyname@shaw.ca, or brought to the Parent Meeting or to the first day of class.

A $50.00 registration fee (First Communion & First Reconciliation) and $30.00 registration fee (Confirmation) covering the cost of books and materials is payable in cash or cheque to Holy Name Parish. Payment can be made at the Parent Meeting or at the first day of class.

By enrolling your child in preparation for a sacrament, please realize that it is your job to learn as much as you can about this sacrament in order to help and support the efforts of the parish in your home. We ask you to continue to reinforce the Catholic tradition as best you can within your family.
